- $217 /Hr. & Callback
- $709/ night call (2.5 hours gratis)
- $2760 / 24 hr call. (4 hours gratis)
- Active unrestricted KY License or IMLC
- Active AHA ACLS certifications
- Active DEA
- Board Certified
- Full-time coverage is mandatory—no deviations from the requested schedule will be accepted.
- Must be able to provide at least 2 supervisory references.
- Clean Malpractice/Background
