Reason for Coverage: Provider is out on LOA

Credentialing Information:

Timeframe: 60 days after receiving completed application (Completed Application needs to be turned around in 5 days or less)

Requirements: CV must be in chronological order beginning at Med School, no gaps over 60 days without written explanation, 3 references

Admissions per day: Varies based on location.  In office 7-10, out of office 5-7 on average.

Patients/day: Same as above

Types of patient cases seen in the clinic and homes: Palliative Medicine patients with life limiting illness, end stage liver disease, cancers, etc.

Is the call only for clinic patients? the call is only for our patients

Are there any visits to a care facility to see patients? We go to most local care homes and facilities in the tri city area

Any Required procedures?  The only procedures would be occasional joint injections, and ear lavages

What are the duties of any interim provider (ex.,locums)?  See patients, refill prescriptions, manage messages for whichever provider is being covered and patients that they are seeing. Call hours vary; some are split into shifts on days, weekends are typically one provider for the whole day, and these are outpatient calls only
