JOB INFORMATION:
• Vaccination Requirement: Required with Booster
• Specialty: Pediatric Nurse Practitioner / Physician Assistant
• Dates of Coverage: ASAP – 6 Months with option to extend, as needed

 Practice Description:
• Setting: Clinic Patients
• Daily Responsibilities: Combination of well exams – newborns – 17 years old; Remainder sick visits
• Shifts/Schedule Expectations: Monday – Friday 8:30a-5p
• Call Schedule/Responsibilities: No Call
• PPD/Census: Estimated 20-24 Patients per day – Provider scheduled 10 patients per 4 hour block & 2 Phone Appointments
• Number of Providers Per Shift & Support Staff: Physicians, other NP’s, and MA’s/LVN’s for Support

Certifications required/accepted: Certified Nurse Practitioner or Physician Assistant
Credentialing Time frame – Are temps available: Estimated 60-90 Days
