◊ Credentialing Timeframe: 60-90 Days (Verify the past 5 years of Employment so if shorter, 60 days is within reach)

JOB INFORMATION:
◊ Total Number of FTE’s needed? 2.0 FTE (1 Fontana 1 Ontario)
◊ Reason for Coverage: Shortage with increase in patient load
◊ EMR: Health Connect 
◊ Coverage Dates (Specific, long term only): ASAP - 3 Months (Through Summer into Fall)
◊ Shift Schedule Details & Shift Duration: Monday – Friday; 8:30 AM – 5:00 PM
◊ Call Schedule/Responsibilities: No call
◊ Type of Patients/Cases/Procedures: Outpatient Clinic Setting – Focus will be primarily OB patients with return OB check up’s. Need to be comfortable with return OB and Ultrasound dating at 8 weeks. Will see some GYN cases as well to fill up the schedule or if patient panels shift. Some Video appointments included along with clinic coverage
◊ Patients per Day/Volume: Scheduled 12-13 patients for morning 4 hours and afternoon 4 hours. Estimated 20-24 patients per day
◊ Support Staff: Full Support RN staff and LVN’s; Supervising Physician will be available

REQUIREMENTS:
◊ What is the minimum length of commitment requirement? Minimum number of shifts required per month? 40 Hours per week – Full Time
◊ Certifications/Qualifications Required (ACLS, BLS, MSQA, TEE, etc.): BLS
◊ Board Certified Required: Certified
◊ IMLC State: No
◊ Active State License Required? Yes
