SCHEDULE:
◊ Coverage Dates and Start Date(Specific, long term only): Start ASAP, pending credentialing
◊ Shift Schedule Details & Shift Duration: Monday – Friday – Clinic Hours 8:30a-5:00p – Some Saturday’s possible with day off during the week
◊ Minimum length of commitment requirement? 13 Weeks but prefer 6 months
◊ Minimum number of shifts required per month? Full Time
◊ Call Schedule/Responsibilities: No Call Required

PRACTICE DETAILS:
◊ Patients per Day/Volume: Typical 11 (4 Hour Blocks) – 22 patients per 8 hours
◊ Type of Patients/Cases: Mainly Adult Population – Chronic Disease Management – Diabetes, Blood Pressure, etc. Standard outpatient primary care/internal medicine coverage. A plus if they can see Peds but won’t see may (Not required)
◊ Procedures/Cases Required: Possible Suturing but no casting
◊ Support Staff: LVNs and MA’s
◊ Responsible of Mid-Level Providers: Supervising Physician on Site
◊ EMR: Health Connect (EPIC)

REQUIREMENTS:
◊ Certifications/Qualifications Required (ACLS, BLS, MSQA, TEE, etc.): BLS
◊ Board Certified Required: Yes
Specific Specialty Board Required? Certified NP
◊ IMLC State: No
◊ Active State License Required? Yes
◊ CSR/DEA Requirements (State or Client): State

◊ Miscellaneous Information: Outpatient Work Only – Monday through Friday with possible Saturday (Day off during week) – If provider only wants M-F, that is fine; Spanish speaking a plus but not required

◊ Credentialing Timeframe: 60-90 Days
