JOB INFORMATION:
◊ Total Number of FTE’s needed? 1.00 FTE
◊ Reason for Coverage: Shortage of Staff / Backlog of Patients
◊ EMR: Health Connect
◊ Coverage Dates (Specific, long term only): ASAP – 13 Weeks, with option to extend, as needed
◊ Shift Schedule Details & Shift Duration: 8:30 AM – 5:00 PM – Monday through Friday but would like flexibility for the providers to work afternoon shifts as well 11:30 PM – 7 PM. Only Monday through Friday’s, no weekends.
◊ Call Schedule/Responsibilities: No call
◊ Type of Patients/Cases/Procedures: Focus will be Well Child Physicals for 9-12 Year old but also will see well baby & child visits & immunizations.
◊ Patients per Day/Volume: Estimated 25-28 Patients per 8 Hour Day
◊ Support Staff: Full Support staff – RN’s, MA’s, LVN’s

REQUIREMENTS:
◊ What is the minimum length of commitment requirement? Minimum number of shifts required per month? Full Time – 40 Hours per week – 13 Week contract with option to extend
◊ Certifications/Qualifications Required (ACLS, BLS, MSQA, TEE, etc.): BLS, DEA
◊ Board Certified Required: Certified
