◊ Shift Schedule Details & Shift Duration: Multiple Shifts – 7a-5p, 3p-11p, 11p-7a – Monday through Sunday

◊ # of ORs currently being run: 5 Different Locations in Fontana/Ontario – 3 Locations in Fontana – 2 Ontario – 2 Open Saturday/Sunday’s
◊ Type of Anes Model and supervision ratio – Care Team Model – 1:3 Anesthesiologist/CRNA Ratio – 70 Anesthesia providers on staff
◊ Case types: Bread & Butter, GS, Orthopedics, GYN, Ophthalmology, Urology, Neuro, Pediatrics (Ages 1-18), Thoracic, L&D/C-Section (May consider providers that can’t do Thoracic, Neuro, Peds, L&D/C Section). All Peds under 1 year old cases handled by Pediatric Anesthesiologist

REQUIREMENTS:
• What is the minimum length of commitment requirement per month? Highly Prefer Full Time – Will ‘consider’ 2 weeks per month minimum
• Certifications/Qualifications Required (ACLS, BLS, MSQA, TEE, etc.): BLS, ACLS, PALS
• Board Certified Required: Yes.
• IMLC State: No
• Active State License Required at presentation? Yes
